For case managers, social workers and organizations placing someone in housing. We will review the referral and come back to you about availability and next steps.
Please keep this form free of sensitive information. Do not include diagnoses, medical or clinical records, medication lists, Social Security numbers or financial account details. If a placement proceeds and additional documentation is required, we will arrange a secure way to handle it.
Referring yourself or a family member? The housing request form is the shorter route.
What happens after you submit
We receive it
The referral is logged and the Eagle Harbor team is notified straight away.
We review
We assess the referral against current availability and fit.
We contact you
We contact the referring organization — you — about availability and next steps.
Placement
If there is a fit, we handle housing details, agreements and move-in arrangements together.
